Any individual who has surfed over the internet must have come across one phrase'BUY VIAGRA'.I sometimes wonder whether we actually know what is viagra.

The basic medicine in viagra,revatio and many more is actually SILDENAFIL,a phosphodiesterase-5 inhibitor.[PDE-5]

Sildenafil has greatly improved the treatment of erectile dysfunction.This drug is effective in a broad range of causes of erectile dysfunction like diebetogenic[due to diabetes],vasculogenic[disorder of blood vessels],psychogenic,spinal cord injury.It is also effective in cases of erectile dysfunction occurring after radical prostatectomy.

Sildenafil is orally effective.It is metabolized extensively in the body.Sildenafil may be administered in the dosage of 25,50 or 100 mg.Onset os action is usually after one hour.
Sildenafil acts by inhibiting an enzyme called phosphodiesterase-5,thus prolonging the action a chemical called nitric oxide.Nitric oxide[NO] is released from the endothelium[lining cells of the blood vessels] and nerves of the penis during sexual stimulation.NO,then causes relaxation of the muscles of corpora cavernosum[a spongy organ present within the penis,which is mainly responsible for penile tumescence or erection] and that of the blood vessels within the penis,thus resulting in penile engorgement and erection.Sildenafil,by a complex mechanism enhances the action of NO,thereby increasing penile tumescence or erection during sexual arousal.

Due to vasodilatation[vessel dilatation[.the side effects sildenafil may cause are headache,nasal congestion,flushing.Impairment of colour vision may also be an adverse effect of sildenafil.

Sildenafil is contraindicated[must not be taken] in individuals with coronary heart disease and those taking medicines like nitrates in any form, be it oral,sublingual,transnasal or topical.Sildenafil enhance the hypotensive effects of nitrates and thus may lead to very low blood pressure or even shock.

It should also be avoided in patients with congestive heart failure,cardiomyopathy.

Sildenafil should be given very cautiously in individuals with liver disease,kidney disease,bleeding disorders.

It usually does not lead to priapism.Caution is required in individuals with sickle cell anaemia,leukemia,myeloma which predispose to priapism.

Above all,it should always be remembered,that whether you buy Sildanefil[Viagra] online or from medicine shop,it should always be taken after a registered medical practitioner has evaluated the individual,and has prescribed the medicine.Please do not take this medicine without proper prescription of a doctor.

EPISTAXIS:TREATMENT

For proper management and treatment of epistaxis, it is very important to know about the mode of onset,duration & frequency of bleeding,amount of blood loss,side of the nose from where bleeding is occurring,whether the bleeding is occurring from front[anterior] or back[posterior] part of the nose.

It is also imperative to know whether the individual presenting with epistaxis has any personal or family history of bleeding tendency.History of known medical conditions like hypertension,mitral valve disease,cirrhosis,nephritis,leukemia etc. and drug intake history like analgesics,anticoagulants like warfarin are also very important for proper management.

FIRST AID

In majority of the occasions,bleeding occurs from the little's area,which is located in anterior part of the nose.This can be easily controlled by by pinching the nose with thumb and index finger for about 5-10 minutes.Application of cold compress to the nose may also help by causing blood vessel constriction.

Cauterisation:This method is useful in anterior epistaxis.After applying local anaesthesia,the bleeding vessel is coagulated with bead of silver nitrate or with the help of electrocautery.

Anterior nasal packing:If bleeding from anterior part of the nose is profuse and/or the bleeding vessel is not visualised then anterior nasal packing may be done.This is usually done with ribbon gauze soaked in liquid paraffin.The pack is usually removed 24 hours after,if bleeding has stopped.If the pack is kept more than 24 hours,then oral antibiotics may be given.

Posterior nasal packing may be required in individuals bleeding posteriorly into the throat.

Posterior epistaxis can sometimes be dealt with endoscopic cautery .

Ligation of blood vessels:

If the bleeding is from the external carotid blood vessel system & the bleeding is not being controlled by conservative measures then in some instances ligation of the external carotid artery may be done.This is usually not done nowadays.in favour of embolisation or ligation of more peripheral branches.

Sometimes in cases of uncontrollable posterior epistaxis,ligation of the maxillary artery may be done.

In occasional instances of antero-superior bleeding,ligation of ethmoidal arteries may be required.

However ligation of blood vessels are required on rare occasions,as most of the episodes of epistaxis can be managed by conservative measures.

The immediate responsibility of an individual suffering from epistaxis & his/her family members ,is to compress the nose as mentioned above,apply some cold compress, and immedietely shift the individual to the nearest health centre.

Note of caution:In elderly individuals hypertension or rise in blood pressure is a fairly common cause of epistaxis.

CARDIOVASCULAR CHANGES IN PREGNANCY

Good morning friends,starting from this post onwards i will try to explain some changes the human body undergoes during pregnancy.Today I'll make an attempt to explain the cardiovascular changes in pregnancy[heart & the circulatory system]

Pregnancy is a hyperdynamic state of circulation.
1.Blood volume increases by almost 40-50%.[peaks in 28-32 weeks of gestation]
2.Plasma volume increases to the tune of 40%.
3.Anemia[due to discrepapancy of the rise of blood volume & plasma volume and also due to iron deficiency]
4.Decreased oxygen carrying capacity of blood.[this is compensated by several mechanisms like increase in cardiac output,decreasing blood viscosity,vessel dilatation etc]
5.Due to the effect of estrogen & progesterone[these hormones rise during pregnancy] dilates the blood vessels so there is a fall in the resistance offered by the blood vessels[systemic vascular resistance & pulmonary vascular resistance]
6.Increase in cardiac output[peaks during 22-28 weeks of gestation].
7.Increase in stroke volume[the blood pumped by the heart with each heart beat].
8.Red blood cell volume also increases to the tune of 20%.
9.heart beat increases during pregnancy by 12-16beats/minute[this may give rise to palpitation i.e becoming conscious about your heart beat]
10.Alteration of blood pressure:Systolic blood pressure and diastolic blood pressure,both fall to a minor extent,but the changes are more marked in diastolic blood pressure.
11.Pedal edema[swelling around the ankles,which pits on application of pressure].
12.Some ECG changes.[ST-T changes]
13.some findings are also observed on auscultation of the heart[examination of the chest with a stethoscope] like murmurs,increased splitting of heart sounds,S3 gallop etc.
14.Decrease in blood viscosity.
15.Shift of the oxygen hemoglobin dissociation curve to the right.
