Friends,i am really very sorry,for quite a few days, i am not being able to visit your blogs.Really very sorry.Going through a multiple of problems.Recently my father has undergone a surgery.The surgery has gone well and he is doing pretty fine.

Secondly,my internet service provider is doing bizarre things.Most of the times i am trying to access the Entrecard blogs,the reply is 'Remote server could not be contacted'.Really irritated with them,decided to change the the service provider.

To end this all my Windows xp has also got corrupted.

No,no that's not the end.being a little emotional person,i sms ed my ex-girlfriend of ten years
to inform about my parent's health....but alas there was not even a single mail or an sms...yes i understood one thing,she has really moved on[and why not three years have gone by]...and myself being an idiot is still walking down the boulevard of broken dreams.I don't really know, why i believe,love happens once,u can be committed only once, and then you have to live with those shattered pieces through out your life....

Yes,then again life goes on,hospital,patients,thesis,parents,friends,me and my loneliness....

Any individual who has surfed over the internet must have come across one phrase'BUY VIAGRA'.I sometimes wonder whether we actually know what is viagra.

The basic medicine in viagra,revatio and many more is actually SILDENAFIL,a phosphodiesterase-5 inhibitor.[PDE-5]

Sildenafil acts by inhibiting an enzyme called phosphodiesterase-5,thus prolonging the action a chemical called nitric oxide.Nitric oxide[NO] is released from the endothelium[lining cells of the blood vessels] and nerves of the penis during sexual stimulation.NO,then causes relaxation of the muscles of corpora cavernosum[a spongy organ present within the penis,which is mainly responsible for penile tumescence or erection] and that of the blood vessels within the penis,thus resulting in penile engorgement and erection.Sildenafil,by a complex mechanism enhances the action of NO,thereby increasing penile tumescence or erection during sexual arousal.

Due to vasodilatation[vessel dilatation[.the side effects sildenafil may cause are headache,nasal congestion,flushing.Impairment of colour vision may also be an adverse effect of sildenafil.

Sildenafil is contraindicated[must not be taken] in individuals with coronary heart disease and those taking medicines like nitrates in any form, be it oral,sublingual,transnasal or topical.Sildenafil enhance the hypotensive effects of nitrates and thus may lead to very low blood pressure or even shock.

It should also be avoided in patients with congestive heart failure,cardiomyopathy.

Sildenafil should be given very cautiously in individuals with liver disease,kidney disease,bleeding disorders.

It usually does not lead to priapism.Caution is required in individuals with sickle cell anaemia,leukemia,myeloma which predispose to priapism.

Above all,it should always be remembered,that whether you buy Sildanefil[Viagra] online or from medicine shop,it should always be taken after a registered medical practitioner has evaluated the individual,and has prescribed the medicine.Please do not take this medicine without proper prescription of a doctor.

VINEFIRE:INDEPENDENT REVIEW

Today,i found an interesting site,it's called the vinefire.com.It appears to be a social bookmarking site along with additional earning opportunity.Vinefire at present is in the beta phase,they have a plan to be acquired by a larger company,and they claim to start payment after that.

They are offering $.54 for each link one follows,$.90 for sponsored links.Everytime someone up votes your posted link, they offer you $.04.Vinefire is also offering referral opportunities.They are adding $10 into your account for every person you refer & 50% of your friend's earning.

If you ask me this payment scheme is too good to be true & rather they have not started paying yet.I don't know what will happen of this site in the days to come.
I am not really a believer of anything coming easy in life.

But the aspect that interested me, about Vinefire is that ,by posting links in this site,you really can get a substantial increase in traffic,to your site.
But here lies another problem,the money-making links are doing better than other links.As most of my links are related health and disease,they are not doing that good but,the i guess the blog authors,who specialize in money making ideas,affiliates,social marketing sites can get a lot of traffic from this site.

My final inference is that let's not think about the money aspect.Considering the traffic problem it appears to be a good site to join.

I shall be highly obliged if you join with my link.

ENDOMETRIOSIS

Hi,friends,today i'll post a brief review on ENDOMETRIOSIS.Endometriosis is a very common disease in women of reproductive age group.It is an enigmatic condition because the exact cause and pathology is not clearly known.

Endometrium is the lining of the uterus,the female reproductive organ where the baby[fetus] grows.Endometriosis is a condition where this endometrium with it's glands and supporting tissue[the stroma] is found in organs outside the uterus.There is a related condition called ADENOMYOSIS,where the endometrial tissue is found in the muscle layer of the uterus[myometrium]

Endometriosis is an estrogen dependent condition,so it occurs more in the reproductive age group.But in some instances,it may also occur in post-menopausal women who are on estrogen replacement therapy.This disease is more common in women who have family history of this disease.

Ovary is a very common location where extra-uterine endometrial tissue is seen.Other locations where this tissue may be found are umbelicus,scars in abdomen.rectum, bladder[although rarely],thorax[chest where lung is located],parts of gut[namely caecum,ileum].This disease causes extensive adhesions in the pelvis[the part of the body, where female reproductive organs,rectum,urinary bladder are located].These adhesions distorts the anatomy of pelvic organs,thus causing a variety of symptoms.

CLINICAL FEATURES:1. pain associated with menstrual bleeding.[dysmenoorhea]
2. pain during sexual intercourse[dyspareunia]
3. chronic pain in lower abdomen .
4. acute exacerbation of pain
5. during menstruation, bleeding may occur from several organs[bleeding along with urine{hematuria};bleeding with stool{hematochezia}].Rarely there may be bleeding along with cough[hemoptysis],if endometriotic tissue is located in the lungs.
6.last but not the least,endometriosis is a very common cause of infertility

CAUSE OF ENDOMETRIOSIS:CAUSE IS NOT PROPERLY UNDERSTOOD.SEVERAL THEORIES ARE THERE.Endometriosis may occur due to passage of endometrial tissue during menstrual bleeding from the uteus into the abdominal cavity via the fallopian tube.It may also occur due to spread through blood vessels and lymph vessels.I'm not going into much details of the causes as it will be a little disinteresting for most of my friends.

INVESTIGATIONS:Although many investigations can be performed, the most relevant investigations are TRANSVAGINAL ULTRASONOGRAPHY[heps to detect ovarian involvement,which is called the chocolate cyst of the ovary].LAPAROSCOPY IS THE MOST USEFUL INVESTIGATION.It helps in the detection of endometriotic lesions in the abdominal cavity,which appear like bluish black nodules[powder burn appearance]It also helps to detect the degree of adhesions in the pelvis.

TREATMENT:MEDICAL:1.COMBINED ORAL CONTRACEPTIVE PILLS.

2.PROGESTATIONAL AGENTS:medroxyprogesterone acetate[oral& injectable preparations are available} & gestrinone.

3.GNRH ANALOGUES:leuprolide & goserelin ;These drugs may cause reduction in bone density[osteoporosis] & symptoms like hot flashes,vaginal dryness.To counter these effects drugs like progestins,tibolone,biphosphonates may be given along with GNRH ANALOGUES.

4.DANAZOL:it has some serious side effects like weight gain , fluid retention in the body,acne,deepening of voice,development of fascial hairs[hirsuitism]vaginal dryness,hot flashes etc.

SURGICAL:Surgical treatment depends upon the age,desire for child-bearing,severity of symptoms etc.
1.destruction of the endometriotic lesions & removal of adhesions in the pelvis by laparoscopic approach.
2.hysterectomy with removal of the endometriotic lesions along with removal of the chocolate cyst of the ovary, if present with preservation of the healthy ovarian tissue.This is done in women who have completed their family,but are yet not of that age to undergo total removal of uterus & ovaries.
3.Pre-sacral neurectomy & laparoscopic uterine nerve ablation[LUNA] for relieving the pain.

EPISTAXIS:TREATMENT

For proper management and treatment of epistaxis, it is very important to know about the mode of onset,duration & frequency of bleeding,amount of blood loss,side of the nose from where bleeding is occurring,whether the bleeding is occurring from front[anterior] or back[posterior] part of the nose.

It is also imperative to know whether the individual presenting with epistaxis has any personal or family history of bleeding tendency.History of known medical conditions like hypertension,mitral valve disease,cirrhosis,nephritis,leukemia etc. and drug intake history like analgesics,anticoagulants like warfarin are also very important for proper management.

FIRST AID

In majority of the occasions,bleeding occurs from the little's area,which is located in anterior part of the nose.This can be easily controlled by by pinching the nose with thumb and index finger for about 5-10 minutes.Application of cold compress to the nose may also help by causing blood vessel constriction.

Cauterisation:This method is useful in anterior epistaxis.After applying local anaesthesia,the bleeding vessel is coagulated with bead of silver nitrate or with the help of electrocautery.

Anterior nasal packing:If bleeding from anterior part of the nose is profuse and/or the bleeding vessel is not visualised then anterior nasal packing may be done.This is usually done with ribbon gauze soaked in liquid paraffin.The pack is usually removed 24 hours after,if bleeding has stopped.If the pack is kept more than 24 hours,then oral antibiotics may be given.

Posterior nasal packing may be required in individuals bleeding posteriorly into the throat.

Posterior epistaxis can sometimes be dealt with endoscopic cautery .

Ligation of blood vessels:

If the bleeding is from the external carotid blood vessel system & the bleeding is not being controlled by conservative measures then in some instances ligation of the external carotid artery may be done.This is usually not done nowadays.in favour of embolisation or ligation of more peripheral branches.

Sometimes in cases of uncontrollable posterior epistaxis,ligation of the maxillary artery may be done.

In occasional instances of antero-superior bleeding,ligation of ethmoidal arteries may be required.

However ligation of blood vessels are required on rare occasions,as most of the episodes of epistaxis can be managed by conservative measures.

The immediate responsibility of an individual suffering from epistaxis & his/her family members ,is to compress the nose as mentioned above,apply some cold compress, and immedietely shift the individual to the nearest health centre.

Note of caution:In elderly individuals hypertension or rise in blood pressure is a fairly common cause of epistaxis.
